Your Comprehensive Guide to Alex's Romances on Grey's Anatomy

Izzie and Alex's flame grows from an innocent friendship, eventually becoming one of Grey's strongest romances. While their relationship goes south after Alex's involvements with nurse Olivia, they eventually reconcile, then even get married after Izzie's cancer diagnosis. While we thought they were made for each other, Izzie disappears on Alex after believing that he sabotaged her career. When she returns to Seattle Grace cancer-free, Alex is happy for her, but tells her that he deserves better. Ouch.

Olivia and Alex share a casual fling before she decides to break things off with him in order to pursue George. Unfortunately, she contracts syphilis from him, which she passes to George. In season two, they start having a sexual relationship again, much to Alex's then-girlfriend Izzie's dismay. Years later, they have the world's most awkward reunion when he treats her kid, with Jo learning about Alex's syphilitic past.

Addison and Alex share a short-lived relationship in season three. After lots of sexual tension, they kiss at Joe's bar and later get down and dirty. But Alex isn't ready for a relationship yet, so things eventually fizzle out between the two. Still, Addison leaves an impression on him, prompting him to pursue peds instead of plastics.

Rebecca Pope occupies a tragic and revealing part of Alex's arc in the series. In season three, Alex rescues a pregnant Jane Doe during the dreadful ferry boat accident and nicknames her Ava, as she couldn't remember who she was. She and Alex pursue a brief relationship, in which he tries his best to care for her. Eventually, Izzie convinces Alex to get Rebecca psychiatric assistance even though Alex claims that he's aided his mother in this kind of situation before. Their relationship becomes a thing of the past after Alex concedes that Rebecca, who suffers from borderline personality disorder, needs more help than he can offer.

Yeah, Callie and Alex were a thing for a very hot and steamy second. We don't learn about their little rendezvous until late in the series, though, when on Lecture Day (season six), the ortho surgeon talks about a major reconstruction surgery that she did with Alex. Only Arizona picks up on what happened between them during Callie's speech.

Alex sleeps with Lexie prior to Izzie's unexpected return. During the season six shooting, Alex hallucinates after getting shot and mistakes Lexie for Izzie. Their fling doesn't last, since Lexie later serves him with the truth about his visions after he goes around the hospital pretending to be a badass with a bullet in his chest.

Lucy and Alex get off to a bad start and have a rather bitter ending. When Alex first meets the ob-gyn attending, he calls her patient a turnip. She then softens up to him after Meredith puts in a good word about him and they learn about their mutual love of college basketball. Their relationship ends, however, when Lucy takes a job in Africa that Arizona initially recommended to Alex. When she asks him to tell her a reason not to go to Africa, Alex tells her to go to hell. Well, it's fine because he'll meet the love of his life (knock on wood) two seasons later!

Jolex begin as close friends, bonding over their troubled youth at Dr. Bailey's wedding. They finally become a couple after Heather's death, but later face rough roads ahead, especially following his father's arrival in Seattle. He asks her to marry her, to which she declines at first. Alex then finds Jo and Andrew in a compromising position, which leads to him beating up the latter and getting served with assault charges. Shortly after they get back together, Jo pops the big question, lining their home with candles. And then they get married (though only after hooking up in a barn that had a dead guy in it) despite a series of wedding mishaps. Cheers to the happy couple!
